Document Review / Approval Sheet

 

The Document Review/Approval Sheet, commonly called the routing sheet, is internal documentation for applications to external sponsors.  This form, to be completed by a research services office or the principal investigator, is a record for identifying characteristics of a proposal.  It includes, but is not limited to, names of principal and co-principal investigators, sponsor and agency; compliance issues; budget and cost sharing information; annual reporting codes; and authorizing signatures. 

When the form is completed by a principal investigator, the PI should complete items 1-16 and 24-26, attach a detailed budget and the project summary, and obtain signatures of the investigators, department heads, and college deans prior to delivery to the Office of Research Policy and Development Support for approval on behalf of the Vice President for Research.
